1. Entry does not guarantee selection. Entry deadline is 9/12/2019. 2. If teacher selected has already booked a procedure at the time of selection, full credit will be applied to that procedure cost. 3. This contest is valued at up to $595 to be used on a single procedure, and cannot be exchanged for cash or other credits. 4. Nominee must be aware and willing to participate in this contest, and must also agree to Terms and Conditions. 5. Winner must be able to provide proof of employment as a teacher at any public or private school for pre-school through grade 12. 6. Procedure must be booked for no later than November of 2019 at either of the Temple, TX or Kyle, TX studios. 7. In the event of a broken cancellation policy, the winner will forfeit the procedure. 8. Winner must be willing to have photos taken, to be used on social media and for other marketing purposes. 9. You may enter as many teachers as you like, but separate entries are required for each nominee.
